Inquiries
A Lifelink home-care coordinator will respond to any questions and provide additional information about our services and in-home assessments. Services may be arranged at the time of inquiry or following an in-home assessment.
In-Home Assessment
Non-medical assessments are conducted to find a person's functioning capabilities, identify their needs and determine what overall services are required in order to help maintain optimal independence. A Lifelink home-care coordinator will review financial needs, develop care plans, make referrals and arrange for care schedules according to individual needs.
How to Arrange for Service
Office hours are Monday through Friday from 8:30 a.m. to 4:30 p.m. To ensure prompt attention is given to all requests, voice mail is available and checked on a regular basis.
Where Service is Offered
Our main service areas include DuPage County and the northwest suburbs of Illinois, although we serve clients throughout the Chicago metropolitan area. We strive to accommodate anyone who requires service within this geographic area.
How Costs Are Determined
Our rates are based on the type of service needed. Costs are determined and discussed with clients prior to beginning of service.
